Submit a request for public information


Processing requests for information

A request for information is a request from an individual to an information controller to provide public information in their possession. A request for information must contain:

  1. name of the information manager;
  2. the name (title) of the requester, postal address or e-mail address, as well as the number of the means of communication, if any;
  3. a general description of the information or the type, name, details or content of the document requested, if known to the requester;
  4. signature and date, provided the request is submitted in writing.

Procedure for registering requests for information

Requests for information received by the Poltava City Council and the Executive Committee of the Poltava City Council, as information managers, are accepted and registered in the Document Support Department of the Office Management Department of the Office Management and Information Technologies of the Executive Committee of the Poltava City Council on working days from 8:00 to 12:00 and from 13:00 to 17:15 (on Friday until 16:00):

If, for good reasons (disability, limited physical capabilities, etc.), a person cannot submit a written request, it must be issued by the person responsible for information requests, necessarily indicating his/her name and contact phone number in the request, and providing a copy of the request to the person who submitted it.

The request can be submitted in person to the executive bodies of the Poltava City Council or to officials who organize, in accordance with the established procedure, access to public information held by information managers during working hours in accordance with internal labor regulations.

A special place for requesters to work with documents is located in room 139 in the premises of the executive committee of the Poltava City Council, at the address: Poltava, Sobornosti St., 36.

Additional information

  1. Information about the activities of the Poltava City Council, the Executive Committee of the Poltava City Council, documents in the possession of the City Council, the Executive Committee of the Poltava City Council are posted on the official website https://rada-poltava.gov.ua.

  2. The request must specify the method of obtaining the information.

  3. A response to a request for information is provided in the manner chosen by the requester within five business days from the date of receipt of the request.

  4. In the event that the request for information concerns information necessary to protect the life or freedom of a person, information regarding the state of the environment, the quality of food products and household items, accidents, catastrophes, dangerous natural phenomena and other emergency situations that have occurred or may occur and threaten the safety of citizens, the response shall be provided within 48 hours from the date of receipt of the request.

  5. In the event that the request concerns the provision of a large amount of information or requires searching for information among a significant amount of data, the period for considering the request may be extended to 20 working days with justification for such an extension. The requester shall be notified of the extension of the period in the manner specified in the application for information no later than five working days from the date of receipt of the request.

  6. Information is provided free of charge upon request.

  7. If the requested information contains documents of more than 10 pages, the requester shall be notified of this within five working days from the date of receipt of the request, indicating the amount of actual expenses, related to copying or printing of documents, and the details and procedure for reimbursement of such expenses in the manner specified in the application for information. The information is provided within three business days after confirmation of payment of the actual costs.

  8. The request may be denied in the following cases:

  • the information manager does not possess and is not obliged, in accordance with his competence provided for by law, to possess the information for which the request was made;
  • the requested information belongs to the category of information with restricted access in accordance with the Law of Ukraine "On Access to Public Information";
  • the person who submitted a request for information did not pay the actual costs associated with copying or printing documents, in accordance with the Law of Ukraine "On Access to Public Information";
  • The requirements for compiling and submitting a request for information, as stipulated by the Law of Ukraine "On Access to Public Information", were not met, namely, the following was not stated:
  • the name (title) of the requester, postal address or e-mail address, as well as the number of the means of communication, if any;
  • a general description of the information or the type, name, details or content of the document requested, if known to the requester;
  • signature and date, provided the request is submitted in writing.
  1. The refusal to satisfy the request for information must state:
  • surname, first name, patronymic and position of the person responsible for considering the request by the information manager;
  • date of refusal;
  • motivated reason for refusal;
  • procedure for appealing the refusal;
  • signature.
  1. Refusal to satisfy a request for information shall be provided in writing.
